Grants

Grant Criteria

Individual grants are limited to 1 per person and capped at $1000.
Grants are limited to equipment, home modifications, caregiver assistance. (Excluding living expenses)
A quote/invoice will be required.
Grants will be paid directly to the vendor (except in rare approved circumstances).
A questionnaire will be used to ask consistent questions to all applicants.
If the requested item is a covered benefit under their insurance plan, we need the written denial from the insurance company or an explanation of why additional funds are needed.
There is limited funding for the grant funds, please be aware that all that apply will not receive funds.
All board members will vote to approve/deny the grant application
